School Year Transition Program
Program Description: The Transition Program is designed to assist blind and visually impaired youth to be ready to work, go to college, or attend a technical school upon completion of high school.
Programs will focus on vocational rehabilitation services that include the following:
Adjustment to Blindness Counseling (Individual or Family), Information and Referral of Complementary/Collateral Services, Communication Skill Training, Personal Management Training, Home Management Training, Adaptive Aids and Device Training, Management of Secondary Disability, Self Advocacy Training, Assistive Technology Training, Rehabilitation Engineering Services, Optical Devices Training, Non-Optical Devices (lighting, contrast, etc.) Training, Peer or Facilitated Support Group, Community Integration, Work Adjustment/Experience Services, Personal and Social Adjustment Services, Vocational Evaluation, Job Coaching, and Job Placement.
Other services include Intake, Assessment of Need-Orientation and Mobility (O & M), Orientation and Mobility-Travel Training, Assessment of Need-ADL, Pre and Post, Assessment of Need-Assistive Technology, Recreation and Leisure Activities and On-going Support Services.

Days of operation: Programs will be provided in four (4) rotations of nine (9) weeks one night each week (Wednesday, from 4:00 p.m. to 6:00 p.m.). Each rotation (or wheel) will focus on a specific skill set. Each participant must attend a minimum of 75% of the time in order to participate in a field trip at the end of the nine week rotation.
A Saturday program is provided (the 2nd Saturday of each month) for all enrolled youth, including those unable to enroll during the week.
A five-week summer camp program is also available. Summer Transition includes training in daily living skills, a work experience, and recreational activities Monday through Friday from 9:00 a.m. to 3:00 p.m.
