Case Management/Social Services

 

Our Case Manager is responsible for conducting new client intakes and for following up on each client’s progress. The Case manager initially interviews clients for background information to determine needs and to develop service plans to satisfy their needs. Referrals may be made to other community resources outside the Lighthouse. The Case Manager works with staff, clients and their families to facilitate successful rehabilitation toward independence.
